Cardiff and Vale University Health Board provide a multi-visit Community Palliative Nursing support service through the provision of Senior Health Care Assistants (SHCA's). The focus of the service is to provide direct care for patients with palliative and end of life needs wishing to be cared for and die within their own homes. The (SHCA's) will work with the District Nursing service who will provide the qualified staff to support the patient and manage any clinical symptoms. The service will provide a multi-visit service based on individual need over the 24hr period. The service will cover the 3 localities: Cardiff South & East, Cardiff North & West & Vale of Glamorgan, including Western Vale.
